Instructions
Welcome to our screening process! To begin, please give us a call to schedule a 15-minute phone screening. If your screening confirms a valid diagnosis, you'll gain access to our client portal where you can complete the Intake form. After that, you'll receive an Assessment form to fill out. Once we review both forms, we'll set up your appointment, which can be in-person or virtual based on your availability. Don't hesitate to call to start your admission process!
